RELEASE OF LIABILITY, WAIVER OF CLAIMS, AND INDEMNITY AGREEMENT
In consideration of being permitted to participate in the 2nd Annual A Tee L Golf Charity Event ("Event") to be held on October 24, 2026 at Country Club of Gwinnett, Duluth, Georgia, I, on behalf of myself and each member of my registered team, hereby agree to the following:
1. Assumption of Risk. I acknowledge that participation in golf and related activities involves inherent risks, including but not limited to bodily injury, property damage, and death. I voluntarily assume all such risks associated with participation in the Event.
2. Release of Liability. I hereby release, waive, discharge, and covenant not to sue A Tee L Golf, its organizers, sponsors, volunteers, officers, and agents (collectively, "Released Parties") from any and all claims, demands, losses, damages, or causes of action arising out of or relating to participation in the Event, whether caused by negligence of the Released Parties or otherwise.
3. Indemnification. I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to participation in the Event.
4. Medical Authorization. I authorize the Released Parties to seek emergency medical treatment on my behalf should I be unable to do so, and I agree to bear all costs of such treatment.
5. Photo Release. I grant A Tee L Golf the right to photograph and record my participation in the Event and to use such images for promotional purposes.
6. Rules & Conduct. I agree to abide by all rules set forth by A Tee L Golf and Country Club of Gwinnett, and to conduct myself in a sportsmanlike manner throughout the Event.
